Personalized Services

At Manitoulin Centennial Manor, we provide a comprehensive range of personalized services designed to meet the unique needs of each resident. Our dedicated staff ensures every resident receives exceptional care in a supportive, respectful, and nurturing environment.

Nursing Care and Individualized Plans

Our team of Registered Nurses, Registered Practical Nurses, and Personal Support Workers operates under the guidance of the Director of Care. Together, they create and implement individualized care plans tailored to each resident’s specific needs and preferences.

  • Care Plans: A detailed assessment of each resident ensures staff are fully informed about their requirements and wishes. These care plans are developed with input from residents and families upon admission and are updated regularly to reflect any changes in needs.

  • Hands-On Support: Personal Support Workers assist residents with daily living activities, ensuring comfort and dignity in every interaction.

  • Open Communication: Families are encouraged to engage in ongoing communication with staff to stay informed and provide input. The Charge Nurse is always available to address questions regarding care.

Physician Supervision and Medical Services

All residents are under the medical supervision of a physician.

 

  • Choice of Physician: Residents can retain their family doctor or choose the facility’s physician. This decision is coordinated with the Administrator and the medical team to ensure seamless care.

  • Medical History: A thorough medical history is maintained to ensure all care decisions are informed and accurate.

  • Medication Management: Our nursing staff dispenses medications as prescribed by the physician and monitors any referrals to community therapies or services for optimal health outcomes.

Pharmacy Services

Pharmaceutical care is provided by Medi-Systems, specializing in long-term care services.

  • Clinical Consultant-Pharmacist: Regular visits ensure each resident’s medication regimen is reviewed and optimized for safety and efficacy.

  • Advanced Packaging: Medications are dispensed in specialized strip packaging for accuracy, with systems in place to monitor allergies and interactions.

  • Billing Support: Most medication costs are billed to the Ontario Drug Benefit plan or other drug plans, with clear statements provided for any uncovered items.

Activity Programs 

The Activity Department provides a variety of programs that recognize the residents’ physical, intellectual, emotional, social and spiritual needs. The department staff is usually available from 8am to 5 pm plus 2 evenings a weekend. And 9 to 5 on Saturday.


Examples of regularly scheduled activities include: Group exercises, Intellectual games Discussion groups, Music therapy, which includes sing alongs and entertainment from the community, easy listening music on tablets for personal use with headphones.


Games that include cards, bocce ball and floor curling and socials, which include happy hour. Bingo every Monday afternoon is a staple! We have birthday parties, special events parties and religious services. Some of these activities occur in the Activity Room/Dining Room on the main floor and may occur in the lounges on each floor. We do many Activities outside in our fabulous courtyard including campfires and we roast marshmallows and hot dogs.

Every Month we hold a Resident Council meeting as well as a Food Committee meeting where residents can come chat and voice any concerns and give input into future programs.

 

Restorative Care

Our Restorative Care program is designed to maintain or improve residents’ physical and psychosocial abilities.

  • Personalized Programs: Residents are assessed to determine eligibility for programs such as walking, mobility exercises, heat therapy, and restorative eating.

  • Team Collaboration: Programs are developed with input from the resident, family, and care team, with follow-up care provided by primary caregivers after program completion.

Dining & Nutrition

Our Dietary Department ensures residents enjoy balanced, nutritious meals tailored to their individual needs.

  • Customized Meals: Diet plans consider medical requirements, cultural and religious preferences, and personal tastes.

  • Ministry Standards: Menus meet the Ministry of Health and Long-Term Care guidelines and Canada’s Food Guide standards.

  • Snacks and Supplements: Residents are provided with nourishing snacks and fluids throughout the day, supplemented as necessary by a Clinical Dietician.

 

Meal Times:

Breakfast: 8:30 AM

Lunch: 12:00 PM

Supper: 5:00 PM

Specialized Foot Care

A qualified Foot Care Specialist visits weekly to provide services such as nail trimming, callus reduction, and specialized care. This service is offered on a fee-for-service basis and scheduled every six weeks or as needed.
